Melioidosis the great mimicker of diseases - a case report.

Abstract

Melioidosis, caused by Burkholderia pseudomallei, is endemic in parts of South India and can be difficult to diagnosedue to its varied clinical presentations. Lack of awareness and persistent fever despite treatment may further delaydiagnosis. We report the case of a 47-year-old man with diabetes mellitus and a history of farming who presented withintermittent fever, multiple joint pains, and sepsis. During the course of illness, he developed a soft-tissue abscess.Melioidosis was diagnosed based on the isolation of B. pseudomallei from blood and pus cultures. The patient wassuccessfully treated with intravenous antibiotics during the intensive phase, followed by oral antibiotics during theeradication phase. Because treatment is prolonged, patient education and adherence are essential for successfulmanagement and prevention of relapse
